## Call your regular doctor when you are unsure

Your regular doctor can start with the timing of the ache and a joint exam. Slow soreness during use often fits osteoarthritis. Long morning stiffness with similar swelling in both hands or feet may need a different doctor.

Sudden heat or severe swelling needs quicker care.

## Compare the two common kinds of soreness

Osteoarthritis usually builds over time, and the joint often hurts during use before loosening with gentle movement. An old injury may explain why one joint hurts more than the others.

Rheumatoid arthritis is different because the body's defense system attacks the thin tissue lining the joints. Both hands or feet may swell in the same way, and morning stiffness may last much longer. An exam and blood tests help tell the two apart.

The symptoms guide the visit, but the exam gives the answer.

## Get quick help for sudden changes

A hot, red, very swollen joint can come from gout or an infection. Fever or feeling very ill makes this more urgent, so don't try to work through it at home.

Get prompt care after a major injury, a locked joint, or sudden trouble standing. New weakness or numbness with back pain also needs urgent help, while loss of bowel or bladder control cannot wait.

These signs are not the usual slow osteoarthritis symptoms.

## Choose the doctor who handles the likely cause

Your regular doctor can examine the joint, review medicines, and order an X-ray or blood tests. A rheumatologist is a doctor who treats rheumatoid arthritis and other diseases caused by the body's defense system. A surgeon discusses joints that badly limit sleep, walking, or basic tasks.

Bring your medicine list, scan reports, and notes about when the ache begins. Ask which cause is confirmed and which is still only suspected. Also ask when another kind of doctor is needed.

More than one opinion may help when the cause remains unclear.

The 2020 ACR gout guideline issued 42 recommendations including 16 strong ones: start urate-lowering therapy for anyone with tophaceous gout, radiographic damage from gout or frequent flares; allopurinol first-line including in moderate-to-severe chronic kidney disease; low starting doses; and treat-to-target dose titration to a serum urate below 6 mg/dL. Gout is treatable to target in a way osteoarthritis is not.

In the only randomized trial of total knee replacement (100 patients with moderate-to-severe knee OA), the surgery group improved more on the KOOS4 score at 12 months than the non-surgical group (32.5 vs 16.0; adjusted mean difference 15.8, 95% CI 10.0 to 21.5) - but had four times the serious adverse events (24 vs 6, P=0.005), and 26% of the non-surgical group chose surgery within the year while 74% did not.

The Lancet Seminar on osteoarthritis states that diagnosis is clinically based despite widespread OVERUSE of imaging, that management should focus on core treatments - self-management and education, exercise, and weight loss where relevant - and that surgery should be reserved for people who have not responded appropriately to less invasive methods.
